Lincare’s Disease Management Programs serve patients with a variety of conditions.
Each of these programs is tailored to the needs of the patients they serve, but they all have three things in common – they are designed to help patients learn more about respiratory disease, comply with prescribed therapies, and achieve better health outcomes.
Lincare's Care Check® program is a clinical service designed to meet the needs of patients with early-stage Chronic Obstructive Pulmonary Disease (COPD). Through enhanced patient education, we strive to deliver a higher level of patient compliance with medically-established protocols. We also work with physicians to obtain clinical feedback in a timely manner, which impacts the day-to-day management of COPD and ultimately enhances the quality of life for each patient. This program is a complimentary service that is provided as a standard of care to all Lincare patients in an effort to leverage education and early intervention as a means to slow the disease process.
With congestive heart failure (CHF) currently ranked as the #2 cause of death in the United States, Lincare offers a multidisciplinary tool designed to assist patients and physicians in the effective management of CHF. This program is designed to minimize hospital admissions and improve each patient's self-management of CHF.
SleepEaze is a standard service used to educate and assess patients being treated for Sleep Disordered Breathing with an overall target of increased success rates with patient compliance to prescribed therapies.
Home-based pulmonary education programs from Lincare provide individualized education and support to respiratory patients and their families. At Lincare, we believe education means understanding, ultimately yielding compliance with recommended medical protocols and the best possible outcome for each patient. Our pulmonary education programs seek to benefit the patient through a foundation of carefully-collected research data and medical expertise. Elements of the program include relaxation techniques, coughing and breathing exercises, the provision of support literature and videos, discharge assessment, maintenance and physician follow-up reports.
As a Coumadin® patient you now have the option of checking your PT/INR at home, at anytime. Testing at home is a convenient alternative to going to your doctor’s office to check your INR levels, and frequent self-testing has been proven to reduce the risk of bleeding and clotting. Your doctor will continue to review your PT/INR test results and make any required changes to your dose. 1,2
