Submission Instructions: Once downloaded and completed, Trial and Funding packets can be submitted by:
- Email: please send all required paperwork in one email to NHCOrders@lincare.com
- Fax: fax all required paperwork to 928-556-0709.
Device Trials
Device trials may be needed during the evaluation process, to identify the appropriate speech generating device (SGD). We are happy to provide the devices, communication apps, and accessories needed during the evaluation. When pursuing funding with Lincare, you can keep the loaner device until yours is delivered.
One packet will allow you to request a trial and complete the funding documents needed should you decide to move forward with funding an Expression Series Device. See Instructions above for submitting these forms.
